NATO - MISSION (CIVIL PERSONNEL ASSIGNED TO MILITARY BASES IN ITALY)
Consular jurisdiction for legal residents of Colorado,
Illinois, Kansas, Iowa, Minnesota, Missouri, Nebraska, North Dakota, South Dakota,
Wisconsin and Wyoming.
Civil personnel assigned to military bases in Italy must provide the following
documents:
1. OFFICIAL PASSPORT with a validity that exceed the
intended stay of the applicant by at least three months. Please make sure your
passport has a blank page to affix the visa.
2. Copy of the applicant's Drivers License or State I.D. in order to
verify his/her residency in this Consulate's jurisdiction.
3. Application form,
filled out completely and signed by the applicant with one recent passport size
photograph, glued to the form. Signature must be notarized.
4. COVER LETTER from the competent military department
in the United States requesting a mission visa: the request must indicate name
and surname of the applicant, duties assigned, destination and duration of the
assignment in Italy, date of departure and telephone number of the official
who is signing the letter (Please see note #1 for particular situations),
5. COPY OF MILITARY ORDERS of the person assigned to
Italy.

7. NO VISA FEE IS REQUESTED (please note that if the
applicant is not in possession of an official passport, a
visa fee does apply).
8. Return envelope: Self-addressed and prepaid UNITED
STATES POSTAL SERVICE (Express Mail), if the applicant will not collect the
passport with the visa in person. Any other mail courier will not be accepted.
9. If there are family members, please request instructions
in order to obtain for them a visa for family reasons.
PLEASE NOTE: Within eight days of entry into Italy, you must report to the QUESTURA
(Police Authorities) to obtain a "PERMESSO DI SOGGIORNO" (Residence
Permit).
NOTE # 1
a) If the applicant has been privately offered a job by a Military Base in Italy
he/she can apply for a Mission Visa only if he/she is in possession of the following
documents:
- Passport showing the following endorsement "the bearer is proceeding
abroad as an employee of a Non-Appropriated Fund Instrumentality of the United
States Government"
- Authorization for Permanent Change of Station (PCS) Travel
- Letter of employment from the Military Base in Italy specifying his/her job
description.
b) If the applicant is employed by a private company and has been assigned to
a military base in Italy in the framework of a work contract between the private
company and the U.S. Armed Forces, a letter from the United States Sending State
Office For Italy at the American Embassy in Rome is to be submitted along with
a a letter from the said company in addition to all the documentation listed
above (from #1 to #9).
