FAMILY MEMBERS ACCOMPANYING US MILITARY
PERSONNEL ON OVERSEAS DUTY
Consular jurisdiction for residents of
Colorado, Illinois, Kansas, Iowa, Minnesota, Missouri, Nebraska, North Dakota,
South Dakota, Wisconsin and Wyoming.
Family members wishing to accompany the
principal (military or civil personnel) assigned to a military base in Italy,
must provide the following documents:
- "No fee" Passport with a validity that exceeds
the intended stay of the applicant by at least three months. Please make sure
your passport has a blank space to affix the visa.
- Application
form, filled out completely and signed by the applicant with one recent
passport photograph, glued to the form. Signature must be notarized.
- Military orders of the principal assigned to Italy, including
the name of the family members authorized to follow him/her.
- Cover letter from the competent military department requesting
a visa for family members: the request must indicate name, surname and family
relationship with the principal, destination and duration of the assignment
in Italy, date of departure, telephone number of the official who is signing
the letter.
- Original marriage certificate and child/children's birth
certificate(s) showing parents name, (plus one copy
of each), (the original certificates will be returned to the applicant).
- For applicants under the age of 18, submit a notarized consent
of both parents or legal guardian.
- Flight itinerary.
- No visa fee is required (if family members are non US citizens
and therefore are unable to obtain a "no fee" passport, a visa
fee applies).
- Return envelope. In order to insure the prompt return of
your passport please provide a self-addressed prepaid U.S. Postal Service
(EXPRESS MAIL) envelope. No other courier will be accepted.
IMPORTANT
Within eight days
from entry into Italy, the foreigner must notify the competent Questura (Police
Office) of his/her arrival.
The average processing time for visas
is four to twenty days.
Rush processing is not possible.
